This study was conducted to define the possibility of use to diagnose and treat at a tertiary hospital for the outpatients who were referred by the primary care medical facilities. The study population consisted of 395 patients who visited outpatient clinic of internal medicine, general surgery, orthopedic surgery and neurosurgery department between March 23 and April 10, 1992, with the referral, note and whose medical record could be found. This study method was inspected by the questionaires at first about the test for diagnosis at primary care medical facilities, and then, was inspected by the medical record later about the test for diagnosis at a tertiary hospital. As the results of this study, because the patient who wrote the summary about diagnosis and treatment have lower rate of repeat test at a tertiary hospital, for reduction of loss of time and medical cost, I thought that there were the need the exact summary of diagnosis and treat.
